  • Information Update:  Have you moved? Send us your up-to-date information. This form can be filled out and sent back to us - all while you're online.
     
  • Sign Up As A Participating Charity: This form lets you accomplish two things. First, it's your official sign-up  form to join us as a participant. Second, completion of this form allows us to put your organization's name and information on our Web site.
     
  • Sign Up as a Participating Professional Advisor: Work with us to find new and exciting ways to tell your clients about  charitable planning.  We have designed the program to encourage people to seek the help of a qualified professional advisor when developing their charitable plans. How can you  participate? Return the Participation Agreement as soon as possible, outlining the activities you agree to undertake.  Your name will be added to our database.
     
  • Request a Speaker: This form is used to request a presentations from the LEAVE A LEGACY® Georgia Speaker's Bureau.
